1. Overview
This document provides a complete summary of the disability, life insurance, and accidental death & dismemberment (AD&D) benefits offered through The Hartford. These programs provide income protection, financial security, and support services for employees and their families during unexpected events such as injury, illness, disability, or loss of life.
All benefits described below are company‑paid, unless noted otherwise.
2. Short‑Term Disability (STD)
Short‑Term Disability provides income replacement when you are unable to work for a temporary period due to injury, illness, pregnancy, or other non‑work‑related medical conditions.
Benefit Summary
- Benefit Percentage: 60% of weekly earnings
- Maximum Weekly Benefit: $1,500
Start of Benefits:
- Sickness: 8th day
- Injury: 1st day
- Maximum Duration: 26 weeks
- Cost: Paid 100% by employer
Eligibility
You are eligible if you work at least 30 hours per week as a full‑time employee.
Guaranteed Coverage
STD is guaranteed issue — no health questions required.
When Coverage Begins
Coverage begins on your eligibility date, provided you are actively at work.
Definition of Disability
You are considered disabled if you cannot perform the essential duties of your occupation due to illness, injury, pregnancy, mental health conditions, or substance use disorders, and your earnings fall within Hartford’s defined thresholds.
3. Long‑Term Disability (LTD)
Long‑Term Disability provides income protection if you are unable to work for an extended period following a serious medical condition.
Benefit Summary
- Benefit Percentage: 60% of monthly earnings
- Maximum Monthly Benefit: $6,000
- Minimum Benefit: 10% of benefit or $100 (whichever is greater)
- Elimination Period: 180 days
- Benefit Duration: Up to Social Security Normal Retirement Age or 4 years (whichever is greater)
- Cost: Paid 100% by employer
Eligibility
Active full‑time employees working 30+ hours per week.
Guaranteed Coverage
LTD is guaranteed issue, with a pre‑existing condition limitation.
Definition of Disability
- First 24 months: Inability to perform essential duties of your occupation.

Coverage Reductions
- Benefits reduce beginning at age 65 and again at age 70.
Portability
Life coverage can be continued if you leave the company through portability or conversion. AD&D cannot be continued.
5. Supplemental / Voluntary Life & AD&D
Employees may purchase voluntary life and AD&D insurance for themselves and their dependents.

- Maximum: Lesser of 3× salary or $350,000

Spouse Coverage
- Increments of $5,000
- Maximum: 50% of employee’s elected amount (up to $25,000)

Evidence of Insurability (EOI)
